Heart to Hand has an established Medical Clinic led by Dr. Robert Delapenha, an infectious disease specialist in the District of Columbia and Maryland. He has been in practice for more than 20 years and is affiliated with multiple hospitals in the area. Dr. Delapenha and our medical staff provide clients with HIV care as well as STI testing and treatment.
Medical Case Management
We help improve the health outcomes of people living with HIV by identifying and removing barriers to medical care, engaging persons in continuous medical care, as well as supporting adherence to treatment regimens. With the help of our Medical Case Managers, as well as Dr. Delapenha, we assist in obtaining insurance and/or medication, navigating medical and behavioral health systems, creating and maintaining a collaborative treatment plan, as well as educating and increasing access to Pre-Exposure Prophylaxis (PrEP).
