Prevention Services
STI/HIV Testing

H2H uses Determine to perform rapid HIV testing, which provides results in as little as 10 seconds. Other STI tests include Syphilis, Chlamydia, Gonorrhea, and Trichomoniasis. HIV/STI testing is performed in-house at our office location. Rapid HIV tests may be performed within the community in our mobile health unit. Our HIV testing walk-in hours are from 10am-2pm Monday through Thursday. Additional STI testing is provided on Monday, Tuesday, and Thursday by appointment only. Outreach and Community Engagement

Heart to Hand can be found at many community events within the Prince George’s and Montgomery County communities, providing HIV testing, health education, and health literacy about HIV and other STIs. H2H engages with multiple community organizations to bring health education and testing services to residents of these two counties. We will soon expand our reach to residents in the community during nontraditional hours. We also accept event requests from community partners within Maryland who would like to provide rapid HIV testing and health literacy. A program of the Washington, DC Department of Health (DOH) HIV/AIDS, Hepatitis, STD and TB Administration (HAHSTA), IMPACT increases the organizations capacity to provide HIV/STI screening, treatment, and PrEP and nPEP among special populations.

Care Services

The Ryan White HIV/AIDS Program is federally mandated and includes primary medical care and essential support services for people living with HIV who are uninsured or under insured. Our Ryan White funded services include Medical and Nonmedical Case Management, Early Intervention Services and HIV Testing, Outreach, and Medical Transportation.

Nonmedical Case Management

Non-Medical Case Management increases access to support services for persons living with HIV. Our Benefit Coordinators assist people living with HIV in addressing social barriers that prevent individuals from obtaining a healthy quality of life – such as emergency financial assistance, medical insurance, transportation assistance for medical appointments, and other services as needed. Our Benefit Coordinators determine eligibility for Ryan White-funded services and facilitates access to benefit programs and other available community resources.

Medical Case Management

Medical Case Management improves the health outcomes of people living with HIV by identifying and removing barriers to medical care, engaging persons in continuous medical care, as well as supporting adherence to treatment regimens. Our Medical Case Managers assist in obtaining insurance and or medication, navigating medical and behavioral health systems, creating and maintaining a collaborative treatment plan, as well as educate and increase access to Pre-Exposure Prophylaxis (PrEP).

Early Intervention Services

The EIS program emphasizes the movement of HIV+ clients along the prevention to care continuum. EIS includes targeted testing, referral services, health literacy/health education, access, and linkage to care. Many of our EIS services are provided through our peer Community Health Worker (CHW) that work directly with clients. CHWs support clients with transportation assistance, one-on-one education related to HIV diagnosis and treatment adherence, accompanying clients to ID and related medical appointments, and home visits to engage individuals who have been lost to care.

Interested persons must go through the Intake process to become a support group member. Please contact the office for more information on how to join support group.

Medical Transportation

H2H can provide transportation for eligible individuals to and from our offices and other medical appointments based on need and availability. Transportation is provided through our in-house van service, car sharing such as Lyft, or Smartrip benefits. Transportation is provided based on a Medical Case Managers evaluation of need.

Medical Services

H2H provides infectious disease care to HIV positive persons at our office. Our resident physician, Dr. Delapenha, has over 20 years of experience in medical care and is affiliated with Providence Hospital. In addition, H2H is affiliated with Physician’s Rx Pharmacy to ensure continuous treatment adherence. Medications can be picked up at our location or delivered to your home.
